As a competent adult, you have the right to decide to accept or refuse any medical treatment. However someday you may become too sick to make decisions about your medical care. If that happens, decisions will have to be made for you. If you have not given any instructions, knowing them though what you have wanted.

News stories have shown us life support systems can keep persons body alive for years, even if the brain is no longer functioning. That's why every adult should have a Living Will and Durable Power of Attorney for Health Care. We are proud to present you Five Wishes, our Living Will Kit which has everything you need to make sure your wishes are known.

You will receive a tool kit designed with help of the American Bar Association‚ commission on law and aging. This kit contains a variety of self-help worksheets, suggestions and resources. There are 10 tools in all, each clearly labeled and user-friendly. The Tool Kit does not create a formal advanced directive for you. Instead, it helps you do the much harder job of discovering, clarifying and communicating what is important to you in the face of serious illness.

This Kit will also include the thorough explanation in layman's terms of the Living Will and durable power of attorney for healthcare. Other features of the Living Will Kit include a section on frequently asked questions and answers about your quality end-of-life documents, making your funeral and cemetery arrangements in advance, suggestions on how to best communicate your decisions to others and information on procedures that ensure your wishes are carried out.
